OT NDIS Functional Assessments

 

What is a Functional capacity Assessment (FCA) ?

FCA is an in-depth report written by an occupational therapist (OT) outlining all of your activities of daily living (ADLs) and the challenges you may face when completing certain tasks. The report is then provided to the NDIS when it comes time for your plan reassessment as evidence and reasoning of certain services required in your new plan. An FCA can also be utilised when applying to be an NDIS participant. 



What to expect?

An assessment is completed between the participant and the Occupational Therapist, either in person or via telehealth. The assessment will include various questions outlining the participant's everyday tasks and any challenges they may face.

Once this assessment is completed, a report is written and sent back to the participant and support coordinator (if they have one). 

The report will be broken down into subcategories of ADLs followed by recommendations and reasoning for services required for your new plan or application to the NDIS.

Why us?

At Rebound Health, our OTs will deliver in-depth reports for your NDIS plan reassessment or application. We have the knowledge and understanding of how services, equipment, home or lifestyle modifications and therapy can assist a person’s capacity to complete ADLs.

Our assessment is a chance to build rapport with the participant and get to know them on a personal level, so the report can reflect their needs and wants clearly.

We have a strong understating of the local services which can assist people, as well as relationships with other allied health professionals if a referral is required.


  • Any relevant information to help your therapist get a good understanding of your situation. This might include your current or previous NDIS plan and any reports/letters from other health professionals including allied health, general practitioners or specialists.

  • You do not need a referral to organise an FCA, however, if you have available funding through your NDIS plan, it would be recommended to utilise this.

  • Your therapist will chat with you about your condition and the impact it has on your everyday life. They might ask about services/treatment you have engaged in in the past and what has/hasn’t worked. You will then collaborate to identify your goals moving forward.
